Crucial Timing Rules

The 7-Month Initial Enrollment Period (IEP)

According to Medicare.gov, your Initial Enrollment Period lasts for a total of 7 months surrounding your 65th birthday month:

Months -3, -2, -1

3 Months Before Your Birthday Month

Recommended Best Window: If you enroll during these 3 months prior to your 65th birthday, your Part A and Part B coverage begins on the first day of your birthday month with zero gap.

Ideal time to enroll
Month 0

The Month You Turn 65

If your birthday falls on the 1st of the month, your Medicare coverage starts the 1st of the prior month! If you enroll during this month, coverage starts the first day of the following month.

* Note the 1st of the month exception rule
Months +1, +2, +3

3 Months After Your Birthday Month

Your last chance to enroll under your Initial Enrollment Period. Under recent Medicare changes, coverage starts the first of the month after you sign up.

Avoid delaying past Month +3!
The Lifelong Penalty Warning:If you don't enroll in Medicare Part B or Part D during your Initial Enrollment Period and you don't have creditable employer coverage, you may face permanent penalties: a 10% Part B surcharge for every 12-month delay, plus a 1% per month Part D penalty tacked onto your premiums forever.
Social Security Administration (SSA) Integration

Are You Already Taking Social Security Benefits?

Your sign-up process depends directly on whether you are currently collecting Social Security retirement income:

Scenario A: Already Receiving Social Security

You are automatically enrolled! The Social Security Administration will mail you your red, white, and blue Medicare card about 3 months before your 65th birthday. Your Part B premium will be automatically deducted from your monthly Social Security check.

Scenario B: NOT Yet Receiving Social Security

You must actively sign up! Medicare does NOT automatically enroll you if you have delayed claiming Social Security benefits. You must apply online at SSA.gov/medicare or call SSA during your IEP.
