Turning 65: Your Complete Medicare Enrollment Roadmap
Turning 65 is your golden window for Medicare. What you decide during this period dictates your healthcare coverage, provider choices, and financial peace of mind for decades to come.
The 7-Month Initial Enrollment Period (IEP)
According to Medicare.gov, your Initial Enrollment Period lasts for a total of 7 months surrounding your 65th birthday month:
3 Months Before Your Birthday Month
Recommended Best Window: If you enroll during these 3 months prior to your 65th birthday, your Part A and Part B coverage begins on the first day of your birthday month with zero gap.
The Month You Turn 65
If your birthday falls on the 1st of the month, your Medicare coverage starts the 1st of the prior month! If you enroll during this month, coverage starts the first day of the following month.
3 Months After Your Birthday Month
Your last chance to enroll under your Initial Enrollment Period. Under recent Medicare changes, coverage starts the first of the month after you sign up.
Are You Already Taking Social Security Benefits?
Your sign-up process depends directly on whether you are currently collecting Social Security retirement income:
You are automatically enrolled! The Social Security Administration will mail you your red, white, and blue Medicare card about 3 months before your 65th birthday. Your Part B premium will be automatically deducted from your monthly Social Security check.
You must actively sign up! Medicare does NOT automatically enroll you if you have delayed claiming Social Security benefits. You must apply online at SSA.gov/medicare or call SSA during your IEP.

Why would someone choose Original Medicare + Medigap over Medicare Advantage, or vice versa? Here is the exact side-by-side analysis:
| Feature | Original Medicare + Medigap + Part D | Medicare Advantage (Part C) |
|---|---|---|
| Doctor & Hospital Network | Nationwide Freedom: See any Medicare-participating doctor, hospital, or academic medical center anywhere in the U.S. No network restrictions. | Local HMO/PPO Network: Must use doctors in the plan's specific regional network. Out-of-network care may not be covered or cost more. |
| Referrals & Prior Authorizations | None required. You can see a specialist directly without referrals or insurance pre-authorizations for covered services. | Often required. Specialist visits in HMOs may need primary care referrals. Expensive MRIs, surgeries, or therapies require plan prior authorization. |
| Monthly Premium Cost | Higher monthly premium: Part B premium ($185.00+ standard) + Medigap premium ($120–$250/mo) + Part D drug plan premium. | $0 or low plan premium: Many plans charge $0 in additional monthly premium (you still pay the federal Part B premium). |
| Out-of-Pocket Bills & Copays | Predictable / Near Zero: Under Plan G, you only pay the annual Part B deductible ($257 in 2025/2026), then 100% of Medicare-covered costs are paid. | Pay-as-you-go Copays: Daily hospital copays, doctor copays, and specialist fees until you hit the plan's Maximum Out-of-Pocket (MOOP up to $9,350). |
| Dental, Vision & Fitness Perks | Not included: Original Medicare doesn't cover routine cleanings, eyeglasses, or gym memberships (must purchase standalone dental/vision). | Frequently included: Routine preventative dental, eye exam allowances, SilverSneakers / gym passes, and over-the-counter credits. |
| The "One-Time Medigap Open Enrollment" | Guaranteed Issue at 65: During your 6-month Medigap Open Enrollment Period starting the month your Part B is effective, insurance companies cannot deny you or charge more for pre-existing conditions. | Trial Right: If you join Advantage at 65, you have a 12-month trial right to switch back to Medigap. After that, in most states, switching to Medigap requires medical underwriting health questions. |
